To reduce your cost for health care and receive the full premium credit for 2021, complete a Personal Health and Well-Being Assessment (PHA) by December 14, at 11:59 p.m. The PHA will not be available for completion starting December 15 until PHA data can be integrated into Workday. The PHA should be available again in early January. Premium credit for 2021 is $25/month for enrolled faculty and staff, or $40/month for enrolled spouses.
