The Ohio State University Foundation,  PO Box 736096, Chicago, IL 60673-6096

AND   Office of Sponsored Projects to:

Ohio State University, Office of Sponsored Programs, PO Box 772398, Detroit, MI 48277-2398- AWD-xxxxx number must be included.

PLEASE NOTE: PUT THE FUND # ON EACH CHECK THAT IS BEING MAILED TO THIS ADDRESS to reduce errors. Papers are getting separated in this mailing process and deposits made to wrong funds as a result.
